The document that proves your child's change of name. Completed by a parent or carer with parental responsibility, prepared by us, and accepted by schools, HM Passport Office and the NHS, or your money back.
Changing a child's name has rules of its own. We prepare the document and set out exactly who has to sign it.
Their current name, their new name and the date the change takes effect, checked by a person before it is printed.
One for the school, one for the GP surgery, one for the passport application. You choose how many.
Who has to sign, in what order, and what to do when more than one person holds parental responsibility.
Your witness must be 18 or over, live in the UK, and be independent of your family.
About five minutes. Your child's current and new name, your details as the parent or carer, and how many copies you want.
A person checks the wording and spelling, then it is printed and posted the same working day if received before 3pm.
Each person with parental responsibility signs in front of an independent witness aged 18 or over.
Send a certified copy to the school, the GP surgery, HM Passport Office and anyone else who holds your child's details.
Your child's deed poll is a signed and witnessed legal declaration. Schools, government offices and the NHS accept it as proof of their new name, subject to our terms. If any of them turns it down, we refund you in full.
Over 900,000 deed polls issued in 13 years.
A school, a GP surgery, a passport renewal, a sports club, a new dentist. Each one asks for proof at a different time. With the digital archive their details stay on file, and another certified copy is a minute's work.
Apply before 3pm on a weekday and the deed poll is prepared today.
